A Vacancy at Pennine Care NHS Foundation Trust.


To actively support the development and delivery of pharmacy services to the Trust’s Crisis Resolution and Home Treatment Teams (CRHT).
To act as a non-medical prescriber in the CRHTs.
To ensure that the specialist clinical pharmacy service provided meets with national standards for the delivery of a pharmacy service.

To develop and deliver clinical pharmacy services in CRHT to nationally approved standards in conjunction with the CRHT pharmacists, technician and pharmacy team thereby ensuring safe and appropriate use of medicines in order to maximise benefit and minimise risk.
To support the CRHT clinical pharmacy team.
To liaise with inpatient wards, community teams and the pharmacy team to ensure smooth transfer of care.
To provide cover for other pharmacists when requested to do so
To work within own scope of competence as an Independent non-medical prescriber within CRHTs.

Main Duties & Responsibilities
