Main Tasks and Responsibilities Clinical 1. Act as a self-directed, autonomous clinical practitioner who can effectively prioritise own workload and that of others in the clinical arena 2. To be a confident, reflective clinician who demonstrates skill in critical decision making and problem solving 3. To possess expert specialist knowledge and demonstrate excellence in clinical competence in a variety of hospital scenarios and settings 4. To respond to track and trigger scoring, enabling timely recognition of deterioration in acutely ill patients 5. To respond rapidly in an appropriate manner to all medical emergency and cardio-pulmonary arrest calls within the hospital setting 6. To effectively perform advanced patient assessment based on history taking and clinical presentation. 7. In liaison with the multi-disciplinary team, initiate appropriate diagnostic intervention and accurately interpret and act upon clinical findings 8. In liaison with the multi-disciplinary team, set appropriate goals for patient management and skilfully participate in the implementation and evaluation of all therapeutic interventions . 9. In collaboration with the multi-disciplinary team, participate in patient stabilisation prior to and during the transfer process to another clinical area. 10. To demonstrate excellent communication skills with all professional colleagues, making appropriate and timely referrals where necessary and working collaboratively to set goals 11. To demonstrate excellent communication skills with patients and their families, providing sensitive, appropriate support and consistent and realistic information 12. To maintain accurate records, effectively documenting patient assessment, care planning, implementation and evaluation. 13. To always practice in accordance with the NMC Code of Professional Conduct and Trust policies and protocols Education 1. To maintain current professional registration with the NMC 2. To maintain current mentor status with the NMC 3. To ensure that own clinical practice is evidence based and that all requisite academic knowledge is obtained and regularly updated. 4. To ensure that all clinical competencies pertinent to the role are successfully achieved and regularly updated 5. To actively participate in the annual appraisal process, demonstrating commitment to the professional development of self and others 6. To devise and implement an educational strategy for the development and assessment of clinical skills and specialist knowledge in ward areas 7. To work collaboratively with colleagues in Learning and Education and other clinical colleagues in the delivery of specialist courses for ward staff within the Trust Leadership and Management 1. To contribute to the ongoing provision and development of critical care outreach services within the Trust 2. To create and maintain effective communication channels both within the outreach team and for the teams interaction with other multi-disciplinary colleagues 3. Provide a safe working environment for outreach team members and ensure sufficient resources are available for service deliver 4. To facilitate teamwork and ensure that the delegation of responsibility to team members is appropriate and managed effectively 5. To effectively manage and evaluate change in clinical practice 6. To work collaboratively with senior multi-disciplinary colleagues within the Trust and attend all relevant meetings impacting on acute care provision 7. To act as a clinical role model and a learning resource for nursing colleagues in ward areas 8. To act as a knowledgeable, highly skilled leader and learning resource for more junior members of the critical care outreach team 9. To participate in the recruitment of critical care outreach team members and provide all new staff with effective induction to the role and workplace 10. To effectively appraise junior team members and facilitate their professional development 11. To effectively monitor junior team members clinical performance and implement supervision and additional training where necessary Research, Development and Improvement 1. To ensure the practice of all critical care outreach team members is evidence based, ensuring current research and best practice guidelines are always implemented 2. To lead clinical auditing of the critical care outreach service and actively participate in research activities 3. To identify areas for improvement, both within the critical care outreach team and in the clinical area, and use recognised quality improvement methods and practices to pursue continuous improvement 4. To identify potential areas for nurse led research within the outreach service and actively participate in research activities 5. To support junior team members in auditing and research activities 6. To lead the presentation of audit and research findings to multi-disciplinary colleagues within the Trust and externally 7. To contribute to the publication of audit and research material nationally and internationally 8. To participate and contribute to regional critical care network activities 9. To contribute to Trust strategies for nursing and clinical governance
