Frimley Health NHS Foundation Trust is seeking a highly skilled and clinically motivatedOutreach Practitionerto join theRapid Response Team.
This clinically focused role supports the implementation and consolidation ofMartha’s Ruleacross the Trust and provides expert leadership, education, and clinical support for patients at risk of deterioration, life-threatening events, or cardiac arrest.
The postholder will bring significant experience in managing medical emergencies and in developing and implementing deteriorating patient and resuscitation policies. Excellent communication skills and a strong commitment to patient safety, quality improvement, and the critical care agenda are essential.

The Outreach Practitioner will be a key member of the Rapid Response Team, focusing on the early identification and management of deteriorating patients to prevent adverse outcomes, including cardiac arrest.
The role involves providing rapid clinical support and expert decision-making during medical emergencies, contributing to the development and implementation of deteriorating patient and resuscitation policies, and working collaboratively with multi-professional teams to enhance patient safety. The postholder will also deliver education and support to staff on early recognition and escalation of care.
