This is an excellent opportunity to join our dynamic community Consultant-led multidisciplinary servicebased at Moulsecoomb Health Centre in Brighton. The team comprises ofDiabetes Specialist Nurses, Nurse Consultant, Diabetes Specialist Dietitians, Podiatrists, Psychologists and support staff supporting adults living with type 1 and type 2 diabetes across Brighton & Hove, High Weald, Lewes & Havens.
This is an 8PA replacement post; the successful applicant will join a team of 3 other consultants (1.86 WTE) alongside a nurse consultant and two resident doctor diabetes placements: a specialist registrar (6 month rotation) and a GP trainee (4 month rotation).
We are looking for a candidate who is innovative and flexible, is committed to multidisciplinary working and will actively participate in developing new approaches to the delivery of high-quality care across our community setting.
Applicants must have valid registration with the GMC and a License to Practice and be on the Specialist Register for Endocrinology (or be within 6 months at the time of interview.)
The service offers a range of modern support and interventions including care planning, personalised goal setting, glycaemic control advice, preconception advice. insulin initiation, structured education programmes, moderate-high risk podiatry and foot care, psychological support, all to help empower patient's self-care and improve their diabetes self-management.
Clinical duties will include an agreed number of outpatient clinics per week. In addition, there will also be a weekly MDT case review and an alternate weekly Primary Care virtual MDT clinic. The Primary care virtual clinic is an opportunity to provide teaching and advice to Primary Care colleagues, as well as to discuss cases.
There are close links with Consultants in Endocrinology and Diabetes at University Hospitals Sussex (UHSx), East Sussex Healthcare Trust (ESHT), and Maidstone and Tunbridge Wells (MTW) to support the management of patients into and out of Level 4 (acute) services.

The post holder will contribute to general and specialist clinics in diabetes for adults and will provide training, support and education to clinical colleagues (internally and externally) and medical students and play an integral role in the development of the service, including research and quality improvement.
The post holder will be expected to work from and travel to any Diabetes service site; the designated hub for the service is Moulsecoomb Health Centre in Brighton. This role will require working closely with a team of diabetes specialist nurses, podiatrist, dietitians and psychologists. Responsibility for clinical advice to other members of the team, advice and guidance to GPs, and triaging referrals will be expected.
Thepost holder will hold an area of lead responsibility (to be decided once in post).
The department is committed to teaching and provides clinical immersion weeks for Y2 Medical students from Brighton and Sussex Medical School, as well as the provision of Diabetes BSMS Modules MDM59 and MDM177.
The department has been involved in a number of research projects. There is an active regional research network and research activity is encouraged.
