KeyClinicalResponsibilities: To be professional and legally accountable for all work undertaken. To work within the standards and guidelines of the trust, the team, professionalservicesand the professional body. To undertake a thorough holistic respiratory assessment of a COPD* patient including but not exclusive to spirometry, medication review, physicalfunctionand any psychosocial factors that may be present. *thismay include patients with ILD, asthma or bronchiectasis in PR andin thefuture,this may be other cardiorespiratory diseases. Toidentifyand prioritise factors involved in a clients presenting needs andidentifyany safety issues through clinical reasoning based on specialist knowledge of current professional opinion andevidence-basedpractice. To ensure that risks areidentifiedand managed as part of this process and that decisions are made with the MDT and support from the immediate respiratory team To set andmaintaina high standard of clinical practice using standardised assessments andvalidatedand reliable assessment tools forinitialassessment and reassessment and evaluation of progress. To use shared decision making to co-produce client goalsappropriate tothem and involving them in the monitoring and evaluation of progress at each step of the way. To deliver individualised exercise programs in line with current best available evidence based on assessment,progressand patient goals. To communicate effectively within the respiratory team and the wider MDT. To attend, activelyparticipate, and asrequiredinitiateMDTmeetings and case conferences,communicatingcontemporaneous feedback of client progress to the referrer and other involved healthcareprofessionals promoting a collaborative approach to patientcare. To use a variety of highly skilled verbal and non-verbal communication and tools to ensure that the client and theircarerunderstandthe relevant individual and group education and advice. This includes clients: whose first language is not English requiring an interpreter with a hearing impairment with altered emotional states with a cognitive/learning/behavioural impairment not understanding or accepting a clinical decision based onsound reasoning. with communication deficits To ensure that staff deal sensitively with users and their relatives /carers with a variety of emotional conditions e.g. depression, anxiety, aggression, frustration, apathy, behavioural and mental health disabilities To provide mediating and negotiating skillsin situations where communication has broken down and to manage conflict effectively To be a specialist /physiotherapy advisor to other professional colleagues workingin the area ofCOPD and Pulmonary Rehabilitation with non-specialist knowledge. To provide specialist contribution to health promotion and health education initiatives within the locality Key Managerial Responsibilities : Toassistand support the COPD Service Lead, taking an active part in steering groups, audit and project leading whenrequired, including National PR Audit and working towards accreditation. Toinitiateand support patient engagement within PR to ensure patient involvement in service development and delivery. To be involved in the recruitment,selectionand induction process for nursing/physiotherapy staff in the COPD service and the wider team asappropriate. To ensure that you achieve andmaintainappropriate levelsof education and continuingprofessional development (CPD),maintainand update a CPD Portfolio, keep up to date with mandatory training, and deliver intervention based on the best available and most up to date evidence. Toassistin the organisation and presentation of In-service training to staff in the team and nurses/physiotherapists throughout the service and be actively involved with student supervision and training. Toparticipatein publicising,promotingandrepresentingthe service. To ensure that all equipment defects,accidentsand complaints are reported to through theappropriate channelsand that all other members of the team are aware if their responsibilities. To develop links with peers in other services, providing written discharge summaries and referrals to other agencies asappropriate. To manage and support others to manage the risks and demands of: Working alone in the domiciliary/community setting Working within restricted spaces Working with inadequate equipment provision Working with patients orcarers/family members who may display aggressive or abusive behaviour or poorly controlled aggressive pets.
