Summary
The postholder will provide efficient and effective rehabilitation support to the Community Independence Service (CIS) Rehabilitation Pathway. As a member of the CIS rehabilitation pathway the postholder will help people to maintain or regain their health and independence at home; enable people to live well, in their own homes, completing roles and tasks that are important to them with as much independence as possible.
The post holder will be emotionally intelligent and have excellent interpersonal and communication skills as the role entails liaising with a wide range of internal and external staff/organisations, often of people in senior roles.

All clinical duties will be carried out under the supervision and direction of a qualified allied health professional such as an occupational therapist or physiotherapist. The post-holder has a responsibility to perform all tasks within the values of West London NHS Trust. Under such direction and with these conditions, the duties will be:
To provide occupational therapy and physiotherapy interventions as part of a specific treatment plan agreed and documented by physiotherapist/occupational therapist.
To undertake tasks in delivering patient care, rehabilitation or social care as directed by the service professionals in a range of settings.
To manage the rehabilitation or care of patients as delegated by the physiotherapist or occupational therapist.
To work independently in the patient’s home following an agreed treatment plan.
To instruct patients and carers, providing direction and guidance on a range of occupational therapy or physiotherapy and self-care/mobility/functional activities, as directed by the occupational therapist or physiotherapist
To undertake skilled support work with patients individually or in a group with or without supervision of an occupational therapist or physiotherapist
