An exciting opportunity has arisen for an experienced and motivated Cardiac Nurse Specialist to join our Community Cardiac Service within the Rotherham Doncaster and South Humber NHS Foundation Trust (RDASH).

You must possess a high level of expertise, relevant cardiac knowledge and skills and clinical competence in caring for patients with complex cardiac conditions. Experience independently prescribing oral anti coagulation, anti-arrhythmic and heart failure therapy in line with up to dare NICE guidance.

To manage a caseload of patients with complex needs, delegating clinical work within the team where possible to ensure that the Band 7 role can respond quickly where complex advice is required by other teams within the community and primary care.



To act as a specialist resource to GP colleagues in providing timely advice and/or clinical intervention to assess and stabilise complex cardiac presentations with a view to safely avoiding hospital admission or admitting to hospital where appropriate.



To provide highly specialist advice and intervention to support assessment, planning and implementation of care, and where appropriate to design and deliver programmes of care using advanced communication skills.



Independent prescriber role and the ability to demonstrate an in depth up to date cardiac evidence base.



To lead, design and implement specialist education and training programmes for other staff and learners including medical staff in primary care ensuring the team provides a specified amount of time to primary and community care each month.
