Summary
We are looking for a highly motivated and dynamic nurse to join our multidisciplinary team as a band 6 Parkinson's Clinical Nurse Specialist.
Our multidisciplinary team currently consists of; elderly care doctors, a nurse and clinical leads for physiotherapy and occupational therapy. Our team works within elderly medicine and has a caseload of patients in York, Easingwold, Malton, Pocklington, Selby and Tadcaster.
We are looking for an enthusiastic and dedicated nurse who has a particular interest and passion in looking after elderly people with Parkinson's. The successful candidate will have excellent communication skills and relevant experience to deliver high quality, specialist care to people with Parkinson's. They will be an integral part of the ongoing development within our patient centred service. In recent years the service has transformed to reflect the needs of our patient group both in and out of hospital.
We are committed to continuous professional development and will offer on-going training within the Parkinson's service. There are established competencies for band 6 Parkinson's Clinical Nurse Specialist via the Parkinson's competency framework. You will be able to use and develop your clinical leadership skills through service pathway innovations that will lead to real quality improvements
It is essential to be a car driver to meet the needs of the role. We would highly recommend arranging an informal visit.
It is an exciting time to join our service. You will be responsible for the assessment and management of people with Parkinson's.
This will include complex history taking, physical examination and ongoing patient assessment. We care for patients in outpatient clinics, during inpatient stays, via telephone consultations and where appropriate provide community visits. The service has an MDT clinic/meeting once weekly.
Your duties will include but not be limited to:
• Possess excellent communication and interpersonal skills. Demonstrating your experience in management of patients with chronic and progressive conditions.
• To work as an autonomous practitioner to deliver an expert nursing service within this highly specialist area
• Ensuring that patients with Parkinson's conditions receive the highest standard of care, with an emphasis on providing expert advice and support to patients and carers, including psychological support.
• To manage a caseload of patients/clients with Parkinson's Disease. Providing professional and clinical leadership and expertise in the specialty
• Ensure that all patients are treated with compassion, dignity, and respect.
• Ensure patients are assessed, have care plans that are implemented and evaluated in accordance with the Trusts and NMC requirements.
