Summary
What we are looking for: Recent experience of providing care and caring for people with life limiting illness or palliative care needs in a health/social care setting. NVQ Level 2 in Health and Social Care or equivalent (preferred, not required). Be person-centred and compassionate when caring for the person and those important to them. Ability to develop and evaluate care management plans that meet the complex needs of the person, and those important to them. Emotional resilience and the capacity to work with a team.
