Crynodeb
Prescription Management Sign and authorise repeat prescriptions in line with practice policies. Ensure prescriptions comply with best practice, local and national guidelines. Provide clinical oversight on high-risk medications and controlled drugs. Medication Reviews & Reconciliation Conduct structured medication reviews SMRs, ensuring medicines are optimized for patient safety and effectiveness. Reauthorise medications for newly registered patients Carry out medication reconciliation for patients on receipt of consultant letters or following hospital discharge. Other aspects of the role will be variable and within the competence of the successful applicant. This may include the management of blood pressure including the titration of medication or working on audits on the instruction and supervision of the GP Partner .
