Practice Nurse – Streatham (Leadership Role)

A well-established, high-performing GP practice based in Streatham is seeking an experienced, enthusiastic, and highly motivated Practice Nurse to join its friendly and supportive multidisciplinary team. This is an excellent opportunity for a skilled nurse with strong primary care experience who is looking to take on a leadership role and contribute to the ongoing development and delivery of high-quality nursing services.

The successful candidate will combine hands-on clinical practice with a key leadership function, supporting the day-to-day running of nursing services and acting as a senior clinical resource within the practice. Working closely with GPs, practice management, healthcare assistants, and external healthcare providers, they will help ensure patient care is delivered to the highest professional standards and in line with CQC, NHS, and clinical governance requirements.

Key Responsibilities
  • Deliver safe, effective, and patient-centred nursing care within a busy primary care environment


  • Act as a senior member of the nursing team, providing leadership, support, and clinical guidance


  • Support the management and coordination of nursing services within the practice


  • Mentor, supervise, and support the professional development of junior nurses and healthcare assistants


  • Lead and manage chronic disease clinics, including diabetes, asthma, COPD, hypertension, and cardiovascular disease


  • Undertake immunisations, vaccinations, travel health (if trained), cervical screening, wound care, and NHS health checks


  • Provide health promotion, lifestyle advice, and preventative care to patients


  • Ensure robust infection prevention and control procedures are maintained and monitored


  • Actively contribute to clinical audits, QOF performance, and quality improvement initiatives


  • Support the implementation of new services, protocols, and evidence-based practice


  • Maintain accurate and timely clinical records using electronic patient systems


  • Work collaboratively with the wider multidisciplinary team to improve patient outcomes and service efficiency


    Essential Criteria
    • Registered Nurse with current NMC registration


    • Proven experience working as a Practice Nurse within a primary care setting


    • Demonstrable leadership, senior, or supervisory nursing experience


    • Strong clinical decision-making, organisational, and time-management skills


    • Excellent communication skills and the ability to lead by example


    • Ability to work independently while supporting and guiding others


      Desirable Skills and Experience
      • Formal qualifications or extensive experience in chronic disease management


      • Experience in staff training, mentoring, appraisal, or clinical supervision


      • Knowledge of QOF, CQC standards, and NHS primary care frameworks


      • Experience contributing to service development or quality improvement projects
