Summary

Location: Across East Lincolnshire (Louth, Market Rasen, Boston and Skegness Community Wellbeing teams).

About the Role:

Lincolnshire County Council’s Adult Care and Community Wellbeing Service is renowned for its high-quality support, having been rated GOOD across the board by the Care Quality Commission. We are seeking dedicated and compassionate professionals to join our Adult Care and Community Wellbeing Service and have a number of Social Worker 2 vacancies throughout East Lincolnshire. These positions have become available due to internal promotions, presenting an excellent opportunity for career advancement within Lincolnshire County Council. You will work under the direction of the Team Manager and Advanced Practitioners to empower individuals who require care and support. Your primary goal will be to identify and build on the strengths and capabilities of individuals, helping them to achieve their desired outcomes and maximize their independence.



Key Responsibilities:



Manage a varied caseload with supervision and advice from the Team manager/ Advanced Practitioners, ensuring your practice is strengths-based and person-centred.
Supervisory responsibilities for Community Care Officers within the teams.
Undertake needs assessments in line with the Care Act 2014, focusing on identifying outcomes, opportunities for independence, and strengths-based solutions.
Collaborate with specialist safeguarding colleagues to conduct safeguarding enquiries, ensuring the person and their desired outcomes are at the centre of practice.
Identify non-eligible and non-social care needs, such as health care or housing, and work with partners to address these needs through information and advice.
Develop cost efficient care packages in collaboration with individuals, carers, and other agencies, ensuring risk is minimised and quality of life is improved.
Work with individuals, families, carers, and communities to help them make informed choices and decisions, enabling them to clarify and express their needs.
Utilise community resources and support networks to meet identified needs and promote social inclusion.
Research available resources and services, identifying and reporting any gaps in service provision.
Maintain and update case notes and other records, write reports as required, and provide evidence in court if necessary.


About You:



Social Worker Level 2: A professional qualification in Social Work with current Social Work England Registration and a minimum of 2 years post-qualification practice experience



In addition, you will:

be able to identify needs and tailor care and support to build on individual strengths. Strong interpersonal, verbal, and written communication skills. Effective IT skills and ability to manage conflicting priorities.
have knowledge of relevant health and social care legislation. Understanding of risk management and developing solutions to manage risks while promoting strengths.
demonstrate values that promote wellbeing, challenge oppression and discrimination, and empower people to live well.
