Come and join the team at GSTT in the role of Lead Cardiovascular Pharmacist - Heart Failure

This is an excellent opportunity for an aspiring and experienced clinical pharmacist to lead our specialist pharmacy service to Heart Failure patients across all sites of the Trust. The post holder will provide leadership for our well-established pharmacist-led medicines optimisation clinics and further develop the service to incorporate remote monitoring for suitable patients alongside our newly formed remote monitoring hub.

The post holder will work closely with the Lead Pharmacist for Cardiology to ensure that the services are developed in a strategic and forward-thinking manner and be provided with ample personal and professional developmental opportunities. The post will provide a dynamic link between clinical practice and service development by creating new models for delivering care and ensuring the best outcomes from pharmaceutical care are obtained.

The post holder will act as a role model for the wider pharmacy team and be a pharmacy expert in this field to ensure a consistently high clinical pharmacy service in line with corporate objectives and the NHS long term plan.

As lead cardiovascular pharmacist for Heart Failure, the post holder will be responsible for managing all pharmacy aspects of the Heart Failure Service, including governance, quality and safety, education and training, policies, guidelines and medicines usage information. They will also work in collaboration with the local ICB and community Heart Failure services to ensure effective transfer of care between secondary and primary care. They will work closely with the Heart Failure clinical leadership team, attend and contribute to consultant ward rounds, attend multidisciplinary team meetings, e.g. Heart Failure MDT, business meetings and clinical governance meetings.
