The aim of the Intermediate Care Southwark is to provide community based care and rehabilitation to prevent unnecessary hospital admission, facilitate early hospital discharges and promote independence reducing reliance on long term social care services.
The successful applicant will work alongside Physiotherapists, Occupational Therapists, Social Workers, Social Care Assessors, Pharmacists, Doctors/GPs and Rehab Support Workers as part of the multi-disciplinary team.
The nursing role includes the clinical assessment of patients’ conditions and intervention planning for management of blood pressure, pressure care and prevention, continence assessment, medication reconciliation, review and/or administration, urgent wound assessment and management, falls prevention and support therapists to optimise rehabilitation.
These roles would be ideal for enthusiastic Band 6 or experienced Band 5 Nurses looking to develop in the specialist areas of community rehabilitation, admission avoidance, holistic assessment, and integrated care.

Professional / Clinical Responsibilities
• To be competent in using a wide variety of specialised assessment and treatment approaches with adults with a range of conditions, including those with severe physical and cognitive impairment in order to formulate treatment plans, maximise rehabilitation potential and set goals.
Management and Leadership
• To monitor the quality of nursing/rehabilitation services offered and contribute to the development and review of quality measures and performance indicators.
Communication
• To be skilled in communicating, using both verbal and non-verbal means, with clients, carers and families who may have a range of physical disabilities or other barriers to understanding.
• To offer advice and communicate written and verbal information to clients/ carers and agencies including other health professionals regarding a client’s management.
Education, Training & Mentorship
• To educate referrers and clients about the services the team offers.
Audit, Benchmarking & Research
• To monitor the quality of services offered by the team and when areas of development are identified, to implement an action plan to address these under the direction of the Senior Specialist Nurse/ Clinical Specialist.
Personal & Professional Development
• To reflect on practice at all times and to effectively participate in clinical supervision.
• To maintain Nursing and Midwifery Council (NMC) registration and to work within NMC code of conduct.

Professional / Clinical Responsibilities
• To be professionally and legally responsible for own practice and accountable for all aspects of treatment delivered to the client to ensure a high standard of clinical care.
• To work as an autonomous lone practitioner, case managing clients in the community and coordinating their care.
• To carry out timely assessment and rehabilitation of community clients to prevent hospital admission or re-admission and facilitate early discharge home from hospital.

• To deputise in some tasks in the absence of the Senior Specialist Nurse if required.
Communication
• To work in a client centred manner with mutually agreed goals, appointments and rehabilitation programmes.
• To communicate effectively with clients, carers and family members and to develop effective working relationships with the multidisciplinary team and external agencies.

• To induct, supervise, appraise and teach students on placements within the team.
