Responsibilities
Conduct comprehensive assessments of clients' needs and circumstances.
Develop, implement, and review personalized care plans in collaboration with clients and their families.
Ensure compliance with relevant legislation, policies, and best practices in social care.
Collaborate with healthcare professionals, community organizations, and other stakeholders to coordinate services.
Monitor and review the effectiveness of care plans and make adjustments as necessary.
Provide advice, guidance, and support to clients and their families on available services.
Maintain accurate and up-to-date records of client interactions and care management processes.
Requirements
A relevant qualification in social work or a related field, recognized by the relevant regulatory body.
Experience in adult social care, including assessments and care planning.
Strong communication and interpersonal skills to engage with clients, families, and professionals effectively.
Ability to work independently and manage a caseload with competing priorities.
Excellent organizational and time-management skills to ensure efficient case handling.
Knowledge of legislation related to adult social care, safeguarding, and mental health.
Commitment to continuous professional development and staying informed about best practices in the field.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £23.86
