Summary


The admission avoidance team want to recruit to new roles: Specialist Support Workers Intermediate Care, we ae seeking people with a passion for caring people in their own home and prompting independence through collaborative working.

You will need to be able to identify and escalate patient deterioration, work across the community teams to dleverpateint care with the support of nursing and therapy staff and have excellent organizational skills. The Intermediate Care team is passionate about training and education and the successful candidate will be offered teaching on a regular basis and will have the opportunity to develop in these roles.

West London NHS Trust is delighted to have been awarded the contract in 2019 to provide community services in Ealing over the next 10 years. We have formed a partnership (Ealing Community Partners) with Central and North West London NHS Trust (CNWL), The Hillingdon Hospital NHS Trust and a number of other partners to provide community services throughout Ealing.

• To assess a defined range of activities of daily living tasks, prescribe equipment, deliver, demonstrate safe use and assess efficacy of these to support the patient achieve their daily activity as a trusted assessor, to include mobility aids.
• To complete an initial subjective and objective assessment of patients. To use clinical reasoning and utilising assessment and treatment skills to formulate treatment plans as directed by qualified staff within protocols and scope of practice Document your findings appropriately in patient records, escalate to the clinical lead as needed and agree further input from the service.
• To have the ability to handle patients with mobility problems, in an enabling way. This may include the use of hoists, sliding sheets, wheelchairs, mobility aids etc in line with Trust manual handling policy and training.
• To complete final visits to patients, complete discharge letters for patients under your care.
• To identify whether referrals to other community resources and services are indicated and complete these within local protocols.
