Support Worker – Hospital Discharge Service (Sandwell)
Location: Sandwell (covering West Bromwich, Wednesbury, Tipton, Rowley Regis)

Hours: 37 hours per week, rolling rota

Salary: £24,242.20 per annum

Contract: 12 Month Fixed Term Contract

Additional Requirement: Full UK manual driving licence held for a minimum of 2 years and a willingness to drive service vehicles

We are looking for a dedicated and enthusiastic Support Worker to join our 7-day hospital discharge service in Sandwell. If you have a caring, patient, and helpful nature you could make a tangible difference to someone’s life. You’ll work closely with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain their independence.

Our support workers are a friendly face to someone who may be struggling to do things for themselves due to age, illness, or vulnerability. You'll allow people to enjoy healthier, more fulfilling lives. What You’ll Do

Respond to referrals and complete welfare calls and home visits.
Attend hospital wards and discharge units to support patients.
Provide practical and emotional support to aid recovery and independence.
Assess service users’ needs, complete and follow support plans, identify safeguarding concerns, and accurately record case notes.
Signpost individuals to appropriate services and community resources.
This is a hands-on, community-based role where no two days are the same. You’ll need flexibility and confidence working in different locations across Sandwell.

To be a successful Support Worker, you’ll:

Have a full UK manual driving licence.
Have a good knowledge of services provided by the NHS and Social Care.
Contribute to the ongoing improvement of service quality and user outcomes.
Be professional. You can deal with queries in a diplomatic and confidential manner.
Be willing to work flexible hours to meet service needs.
There is no personal care or administration of medication for this role.
Shift Pattern

4-day working week on a rolling rota (3 long days + 1 short day).
Long days: 9:00am – 8:00pm
Short day: 9:00am – 5:00pm or 12:00pm – 8:00pm
3 rest days after each working block.
Includes one weekend every three weeks.
Rotas provided 3 months in advance.
