Crynodeb
We are looking for an extra clinical pharmacist to join our existing pharmacy team to help deliver our medicines optimisation goals. The role will be challenging and varied and will include Conducting face-to-face and remote medication reviews with patients Answering medication related queries from patients and other clinicians Optimising medicines to manage chronic diseases Managing acute medication requests Medicines reconciliation Audit work Liaising with our established PCN pharmacy team You will be supported in terms of learning and professional development through regular practice team meetings on a variety of topics. Although an existing prescribing qualification is desirable, support to achieve this would be provided. There is flexibility to develop the role according to your skills, experience and interests in line with the needs of our practice and patients.
