Their Learning Disabilities Team supports adults to live safely, independently and with dignity, ensuring care planning is strengths‑based, outcome‑focused and aligned with individual needs and aspirations.

The Role
Cardiff Council are seeking an experienced Social Worker to join their Learning Disabilities Team. You will work with adults with a range of learning disabilities, undertaking assessments, reviews and care planning to ensure individuals receive the right level of support to maximise independence and quality of life.

You will work collaboratively with families, carers, health professionals and partner agencies to deliver safe, proportionate and personalised interventions. The role requires strong analytical skills, confident decision‑making and a commitment to promoting wellbeing, safeguarding and positive outcomes.

Key Responsibilities
Undertake strengths‑based assessments and reviews in line with statutory requirements.

Develop, implement and monitor personalised care and support plans.

Work collaboratively with individuals, families, carers and multi‑agency partners.

Identify and manage risks, ensuring safeguarding concerns are addressed promptly.

Support individuals to access community resources, services and opportunities.

Maintain accurate, timely case records and documentation.

Contribute to multi‑disciplinary meetings and best‑interest decision‑making.

Ensure practice aligns with legislation, policy and the principles of the Care Act.

Candidate Requirements
Qualified Social Worker with current registration.

Experience working with adults with learning disabilities.

Strong understanding of the Care Act, Mental Capacity Act and safeguarding frameworks.

Ability to complete high‑quality assessments and person‑centred care plans.

Confident in multi‑agency working and partnership‑based practice.

Excellent communication, organisational and analytical skills.

Ability to work independently while contributing positively to team practice.
