Crynodeb
The role is an integral part of the patient experience and journey in the hospital. The role acts as a main point of contact for patients and their family along with multiple partners to ensure the appropriate care plan is in place for discharge.
The aim is to remove barriers prior to discharge, aiming home first using a strength based approach.
This role is primarily for Ealing Hospital, but the employee will need to remain flexible to cover cross-site when needed
Be responsible tooptimisehospital bedutilisationand reduce the length of stay by ensuring thetimely, safe transfer/discharge of patients to the mostappropriateand agreed environment. This will includemonitoringthe transfer of patients to non-acute rehabilitationcentresandfacilitatingdelayed transfers of care.
Instigate and co-ordinate meetings withpatient, family/carers and community agents andfacilitatejoint working partnership between Primary and Secondary Care.
