Summary
A challenging and impactful opportunity is available for an outstanding candidate to take up the Head of Integration for Discharge planning role at London North West University Healthcare NHS Trust. We are looking for an experienced, organised, effective communicator and leader to join one of the busiest acute trusts in the country.
Timely, safe and well-coordinated discharge of acute patients is one of the most significant challenges acute trusts face, and the successful candidate will provide expertise, leadership, influence and support within a system-owned model of integrated discharge.
The post holder will be responsible for delivering a robust, system-owned model of integrated discharge that ensures effective, person-centred and integrated pathways of care out of hospital are embedded through interdisciplinary ways of working and clear, well managed processes and protocols. They will report and monitor the improvement of operational performance ensuring that
patients are discharged in a timely and safe fashion and act as a system leader when required across NWL to ensure all partners are supporting and delivering discharge
The Integrated Discharge Team is a collaborative and partnership approach to coordinating and planning transfer of patients with complex health and social care needs back to their usual place of residence wherever possible following an episode in hospital.
From a Trust perspective they will improving hospital flow management processes to ensure it facilitates accurate decision making and out of hospital resource allocation, as well as identifying clinical development priorities for the organisation to support flow and improved communication across the multiple clinical teams within the Trust
