Advance Your Compassionate Career: End of Life Care Nurse

Do you possess exceptional communication skills & are driven to ensure every patient experiences dignified, high-quality End of Life Care?

We're seeking a highly motivated & experienced clinician to join our End of Life Care Team in Merton as a Community End of Life Care Nurse. This is a unique opportunity to shape quality palliative care across our community.

Why this role is unique and impactful:
• Pivotal Role in Quality:This isn't just clinical delivery, it's about facilitation, mentorship, and policy implementation. You'll be an essential link, working collaboratively to embed best practice & Coordinated Care across primary care, social services & care homes.
• Expert Autonomy:While supported by the Band 7 Specialist Nurse, you'll work with a moderate degree ofautonomy, utilising your clinical judgement & discretion to provide direct advice & mentorship to front-line staff caring for dying patients.
• Strategic & Clinical Development:This post offers an immediate chance to develop your clinical leadership & strategic skills. You'll be instrumental in managing a patient caseload to facilitate best practice & collecting data to support key performance indicators (KPIs) for End of Life Care.
• Collaborative Environment:You'll be a key partner across the Merton community, working closely with GPs, other specialist nurses, case management teams & the voluntary sector to ensure service sustainability.



If you possess:
• Significant experience and passion for palliative or end of life care.
• A proven ability to communicate clearly, succinctly, and with genuine empathy.
• The commitment to working autonomously and driving patient-centred, evidence-based care.
