Summary
Merton Domiciliary Therapistswill work within Locality Teams as part of the wider Multidisciplinary Team (MDT), providing high-quality rehabilitation to the Merton population.
The post holder will support the rehabilitation of adults living with frailty and complex health needs, helping to optimise their independence, health, and wellbeing, and to prevent hospital admission or premature moves into care homes. We are strongly committed to the assessment and rehabilitation of patients living within the Merton community.
The post holder will have a key role within the team, working with patients, service users, and carers to provide highly specialist Physiotherapy assessments and treatment plans. They will support individuals to maintain or regain mobility, independence, and wellbeing within their own homes, setting goals and reviewing outcomes to reduce the need for long-term support.
The post holder will also provide clinical leadership within the locality, offering advice to Physiotherapists and the wider MDT. They will be responsible for the education and supervision of junior Physiotherapy staff, Assistant Practitioners, Rehabilitation Assistants, and Physiotherapy students.

• Providing a highly specialist Physiotherapy service as part of a multi-disciplinary team, to patients in their own homes and within a clinic setting.
• To provide clinically effective, person centred, and evidence-based Physiotherapy assessment and intervention to patients with complex physical health needs
• To assist the Clinical Lead and Clinical Service Manager in the planning and delivery of Physiotherapy services to the patients, their family/carers, students, Apprentices complying with Health and Care Professions Council (HCPC) and Chartered Society of Physiotherapy (CSP) professional and service standards of practice.
• To provide support and supervision for junior staff and students.
• To act as a practice educator for students on their placement and support junior staff members and colleagues in doing so.
• To assist in the management, development and research of Physiotherapy services, including service reviews. To hold responsibility for defined projects.
• To provide support and training to all staff with regards to Physiotherapy interventions.
• To comply with the HCPC and CSP Code of Ethics and Professional Conduct
• To deputise and lead the service as and when needed in absence of Clinical lead/Operational Manager.
• To lead clinical triage with other senior therapists and clinical Lead in the team.
• To work flexibly and travel to multiple sites as required.
