The candidate would be expected: To carry out biopsychosocial assessments for individuals referred to the service To develop shared biopsychosocial formulations and agree evidence-based treatment plans in collaboration with service users To deliver a range of psychosocial interventions across outpatient and community settings, including home visits To review patient progress continually throughout therapy and adapt plans in response to individual need To work closely as part of a supportive multidisciplinary team, contributing to regular MDT meetings To manage an autonomous caseload, with appropriate support and supervision, ensuring timely and regular contact To contribute to the duty role and screening and triage of referrals into the service To build and maintain effective links with colleagues across Liaison Psychiatry Services and including NICPM, Leeds Teaching Hospitals Trust and Primary Care To provide clinical supervision where appropriate and support the development of others To play an active role in service development, including leading or contributing to service improvement initiatives such as the TRACCS pathway We recruit people based on their values and qualifications (where required) and believe that their lived experience is an advantage, this means that together we are as diverse as the communities we care for.
