We are excited to announce the launch of our new Assertive Outreach Team. This fantastic opportunity will enable you to take part in the roll out and development of our new county-wide service, with two bases one in Peterborough to cover the North of the county and one in Cambridge to cover the South of the county.
Our Assertive Outreach team will work in collaboration with our existing CPFT services to support the patients with the most complex and severe needs. In this role you will provide clinical nursing expertise within an MDT. The service will carry a small caseload to enable intensive community-based mental health care to patients with complex mental health needs who struggle to engage with traditional services.
We are looking for experienced clinicians with an interest in working in an innovative and developing service. This role will provide a fantastic development opportunity for a clinician looking to further develop their clinical management skills or someone with an interest in moving into a management role in the future.
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. • We are looking for a clinician to support working-age adults who have complex and severe clinical needs and struggle to engage with traditional services. As a CPN in our Assertive Outreach team you will be expected to provide care co-ordination for a caseload of patients, ensuring that full assessments for individual nursing care needs are completed for every patient and that care is planned, implemented, and reviewed on a regular basis.
• This role will offer you an opportunity to work with a diverse and complex range of patients. We are looking for individuals who can work confidently and compassionately within a multi-disciplinary team. We can support agile working for staff, using appropriate IT equipment and support.
• You will be expected to deliver care co-ordination to a specified caseload operating under the DIALOGUE+ framework. You will have the opportunity to access training and development to support you in your role within the team.

• To always promote the highest professional standards by demonstrating clear management responsibility, through ensuring high standards of care and that the smooth day to day running of this highly complex service is maintained to the highest level.
• To comply with the Nursing and Midwifery Council Code of Professional Conduct or appropriate professional body.
• To set, implement and monitor high standards of professional conduct for all staff.
• Be responsible for the assessing, planning, implementing and evaluation of all patient care using CPA / DIALGUE +framework and leading staff to provide the highest standards of care.
• Take a lead role in ensuring nursing input to multi-disciplinary meetings.
• Ensure the reports provided by the clinical staff to support specialist placements for those with complex needs and for Mental Health Act tribunals and managers hearings are of a high standard and provide the necessary information in a timely way.
• Provide specialist consultation and advice to the nursing team, multi-disciplinary team, clients and their families, user groups, statutory and non-statutory agencies.
• Adopt an appropriate style with staff for achieving group objectives monitoring and evaluation workloads. Providing vision and inspiration through effective clinical leadership to ensure that the skills and competencies of staff are actively developed.
• Be conversant and proactive with Government legislation pertaining to the Mental Health Act, CPA/ DIALOGUE +, National Service Framework, Clinical Governance and NHS Plan.
