Crynodeb
We are looking for a compassionate, motivated Rehabilitation Assistant to join our team. In this role, you will provide both general and condition-specific support to a diverse group of patients, helping them achieve their rehabilitation goals in a safe, inclusive, and person-centred way.
You will work closely with registered nurses, general practitioners, and allied health professionals across a variety of settings, including our community rehabilitation unit and in patients’ and carers’ homes. Collaboration, good communication, and respect for each individual’s needs and background are key to success in this role.
• Deliver high-quality rehabilitation support under the guidance of allied health professionals.
• Assist patients with personalised care and rehabilitation programmes, promoting independence and wellbeing.
• Contribute to a positive, inclusive environment where patients and carers feel respected and supported.
• Work alongside the Outreach Rehabilitation Team to help ensure smooth, well-coordinated discharges and continuity of care.
• Maintain accurate documentation and actively participate in service improvement.
• About You
You hold a Foundation Degree in Health and Social Care (or equivalent experience or qualifications).
• You are committed to developing your skills, including completing role-specific competency assessments.
• You value equity, dignity, and patient choice in all aspects of care.
• You are adaptable, reliable, and confident working in a range of community settings.

• As a Rehabilitation Assistant, you will support the delivery of high quality patient care by managing your own caseload of appointments, assisting with rehabilitation programmes, and working closely with therapists, nurses, and other health professionals.
• You will communicate regularly with patients, carers, and the multidisciplinary team, monitoring patient progress and promptly reporting any changes in condition.
• You will contribute to service quality and improvement by supporting audits, maintaining clinical supplies, ensuring equipment is used safely and efficiently, and following Trust policies and clinical protocols.
• The role also involves accurate record keeping, participation in team meetings and training sessions, and the delivery of patient education, including leading group sessions once trained.
• You will use strong communication skills to motivate and support patients from diverse backgrounds, adapting your approach to meet individual needs and promote patient centred care.
