Support required with accessing the community
Preferred days/times:
Support required in the morning, time and day to be arranged
Tasks/duties:
Support needed for the client to access the community e.g. go to parks/green spaces, go for a coffee and/or go shopping
PA will need to have access to a vehicle and a clean, full, UK driving licence
Some toilet assistance is required when out in the community
Reassurance may be required to be given at times when out and about
Client main diagnosis:
Asthma
Paranoid schizophrenia
Cerebral palsy
Mild Learning Disability
Peanut allergy – Epipen in place
Manual Handling
May need support to transfer on and off the toilet from mobility scooter or wheelchair when in the community depending on equipment in place in public toilets. In terms of using a car, the client can lower themself into smaller cars and uses the grab handle above the window for support

Communication:
The client can be difficult to understand however as a person gets to know them it gets easier. They become frustrated when not understood
Client enjoys:
The client enjoys going shopping, going to green spaces e.g. parks. They find being in crowded areas stressful.
This opportunity is suited for someone who is responsible, reliable, patient, proactive and respectful of others’ privacy, dignity, values, and beliefs
