Location: PENRITH BUT BASED OUT OF KENDAL OFFICE DUE TO FLOODING
Job Type: TEMP
Shifts: Full time, Mon - Fri
Caseload: 15-30 (approx.) cases - Mixed complexity
Hybrid: Yes - Agile working, request to attend the office for monthly team meetings and 1x day a week (Wednesday - in Kendal) desirable.
About the Role: As a Social Worker, you will provide essential support to individuals and families facing a wide range of challenges. You'll assess needs, develop care plans, and work alongside multidisciplinary teams to safeguard vulnerable people and improve their quality of life. This is a highly rewarding role where you will directly impact the wellbeing and safety of those you support. Key responsibilities include: * Comprehensive Assessment: Conduct detailed assessments and risk evaluations for service users.
* Care Planning: Develop, implement, and review tailored support plans.
* Safeguarding: Ensure the protection of vulnerable children and adults.
* Advocacy: Support clients to access healthcare, housing, benefits, and community resources.
* Collaborative Working: Partner with healthcare professionals, local authorities, and voluntary organisations.
* Documentation: Maintain accurate and timely case notes in compliance with professional standards

- Care Act assessments
- Capacity assessment and Best Interest decision processes
- Reviews of established residential and nursing placements across North Cumbria
- Facilitating changes in placement
- Facilitating hospital discharge processes
- CHC processes - including submission of FNC forms, checklists and attendance/engagement at DST
- S117 Reviews
- Court of protection witness statements
- Applications for deputyship and appointeeship where applicable
