Crynodeb
1. Coordinate referrals and ensure all relevant information is present as per policy and protocols. 2. Ensure patient appointments are booked in line with the pathway protocols 3. To be a point of contact for GPs who have queries regarding their patients that have been referred to the service. Coordinate the necessary assessments, appointments or investigations as identified 4. Support the delivery of patient information to ensure all patients/carers receive appropriate verbal/written communications on a timely manner this may include printing out information documents and monitor all aspects of care coordination and service delivery. 5. Act as the patient advocate and facilitator in order to resolve issues that may be perceived as barriers to accessing the service. 6. Effectively utilise a computer and various software applications in order to enter and maintain patient data, communicate with the multidisciplinary team and complete other IT tasks as required by the post.
