Summary
Main Duties and Responsibilities: 1. To assess and manage mental health needs, including risk, to create care plans with the individual that will lead to recovery whilst supporting them with a safety plan. 2. To act as the duty lead on shift and allocate work to other team members as appropriate, to run the shift in a safe and effective way. 3. To manage and prioritise work that is allocated to you, to complete the assessment of mental health and risk and record on electronic systems as needed. 4. To be able to supervision and caseload mange more junior staff. 5. To take referrals over the phone, via fax or by post. 6. To work in conjunction with other professionals E.g. GPs, Police, Ambulance, Housing 7. To works in partnership with the individual to develop a plan of care that the service user agrees with. 8. To review the effectiveness of the care plan where appropriate. 9. To make a speedy and appropriate response to all work referred. 10. To maintain client records in accordance with Departmental requirements and to ensure that they are properly stored and available to the Line Manager at all times, including the appropriate use of Information Technology. 11. To undertake regular supervision or consultation with the Line Manager and to ensure that the Team Manager is made aware of any issues of major importance or concern. 12. To carry out such other duties as may be required from time to time and which are commensurate with the grade of the post. 13. To develop good working relationships with other agencies and other colleagues. 14. To promote anti-discriminatory and non-oppressive practice and service delivery.
