Are you driven to make a real difference in people’s lives from patient own homes? We’re looking for a compassionate and proactive Band 6 Deputy Sister/Charge Nurse to join our well-established York Community Response Team, covering York and the Shar Locality.

What You’ll Do:
• Deliver high-quality holistic care to prevent unnecessary hospital admissions, alongside specialist nursing interventions
• Support timely discharges and promote recovery at home
• Collaborate with a multidisciplinary team including Physiotherapists, OTs, Nurses, and Support Workers
• Engage patients and families using health coaching techniques to set realistic, empowering goals

The post holder will co-ordinate activities in the CRT team ensuring effective communication is always maintained, providing support to registered and unregistered staff within the ward/department.
This post will play a key role in the assessment of patient care needs and the development, implementation, and evaluation of programmes of care.
The post holder will act as a role model, promoting quality evidence-based practice, developing the nursing team, and ensuring effective management of the ward/department in the absence of the Team Leader.
The post holder will work in partnership with the Team Leader and the team to ensure that high standards of nursing care are always provided.
The post holder will provide day to day support to the non-registered clinical support team, consisting of Nurse Associates and Generic Support Workers within the York Community Response Team.
