Crynodeb
Support patient flow from acute care to specialist rehab and community settings for patients with complex rehabilitation needs.Collaborate with providers to efficiently use rehab beds and resolve bottlenecks.Identify barriers and unmet needs using data and highlight areas for development.Act as a link between providers and commissioners, making recommendations to improve health equity, service quality, and resource sustainability.Provide patient-specific advice and monitor adherence to care pathways.Ability to travel to all site across the SE regionThe successful candidate will require:Highly effective communication skillsData analytics and processing abilitiesClinical credibility, advanced clinical decision making skills and ability to support clinical teams with education in all settings.
