The South East (SE) Neurosciences and Trauma Strategic Network is seeking a Complex Case Manager (CCM) to support the complex rehabilitation ODN and the Strategic Network in reviewing and transforming needs based complex rehab pathways across the South East.

The successful candidate will work with 2 existing CCM's, with this role having a particular focus across Hampshire and Isle of Wight.

This role is unique in that it supports patients and their families in navigating the highly complex rehabilitation pathways and supports the Strategic Network in its transformation / streamlining of the pathways to improve access and outcomes for complex rehab patients.


This roles reports to the lead of the Neurosciences and Trauma Strategic Network and will support the complex rehab pathways across all the ODN's within the Strategic Network.

Support patient flow from acute care to specialist rehab and community settings for patients with complex rehabilitation needs.
Collaborate with providers to efficiently use rehab beds and resolve bottlenecks.
Identify barriers and unmet needs using data and highlight areas for development.
Act as a link between providers and commissioners, making recommendations to improve health equity, service quality, and resource sustainability.
Provide patient-specific advice and monitor adherence to care pathways.
Ability to travel to all site across the SE region
The successful candidate will require:
Highly effective communication skills
Data analytics and processing abilities
Clinical credibility, advanced clinical decision making skills and ability to support clinical teams with education in all settings.
