Job Purpose To work as a member of the Specialist palliative & End of Life care team as part of the wider primary and secondary multi disciplinary teams and the independent sector to ensure a cohesive holistic and evidence-based approach to the management of Specialist palliative & EOL care patients Our Shared Values and Behaviours We are committed to delivering outstanding care and creating a culture where everyone feels respected valued and supported We seek individuals who share our purpose of helping people live healthier more fulfilling lives and who are motivated by our mission to be the best team we can be You will be expected to uphold our values by communicating openly acting with kindness showing respect for others and taking responsibility for your actions These values guide how we work together and continually improve the care we provide Our values are Being open and honest we all communicate clearly and honestly asking for help when we need it and making it easier for the people around us to share ideas and concerns Ensuring people feel cared for we all take responsibility for actively supporting and nurturing a kind and compassionate environment for ourselves and others Showing respect for everyone we all act with consideration and fairness valuing each other as individuals and appreciating our different perspectives Clinical and Professional Constantly practices autonomously by assessing and effectively prioritising referrals to the service to deliver patient focused appropriate care in the context of a whole service approach Provide a specialist and holistic Specialist palliative/EOLC care assessment of patients and their carers needs following referral which realistically minimises physical symptoms and the social financial emotional and spiritual implications of the disease process Provides highly specialist care and advice for patients and carers and assists the multidisciplinary team in delivering and evaluating the interventions plan of care reviewing and amending this care as appropriate Frequently acts as a primary resource for site-specific specialist nurses nursing medical teams and the multi disciplinary team Regularly assesses patient for inpatient admissions to specialist palliative care level 2 bedded units Has admission rights to palliative care inpatient unit in liaison with Consultant in Palliative Medicine Frequently initiates and participates in highly sensitive and complex communication issues with patients carers nursing medical teams including pre bereavement support advice and imparting life changing news Regularly reviews and develops appropriate patient documentation in line with Trust policy and Department of Health recommendations To offer a nurse prescribing service to palliative care patients in line with Department of Health and Acute Trust guidelines which will reduce patient and carers distress by being able to prescribe immediately and appropriately in response to patient needs Uses an Electronic Patient Record & palliative care database To participate in a 7 day palliative care visiting service that includes weekends and bank holidays Education Constantly acts as a resource for information on specialist palliative & EOLC care within the Worcestershire Acute Hospitals NHS Trust disseminating evidence-based knowledge and skills to clinical staff at levels Educates patients and their carers in the understanding of their illness treatments and provides other sources of information help and support Participates in county wide palliative education initiatives and development for example ICB End of life Strategy and Gold Standard Framework initiative Provides placements and mentorship for professionals studying palliative & EOL care undergraduate and postgraduate level Develops and produces written information for patients carers and professionals on palliative care services To identify training needs, design and develop an ongoing programme of training in the use end of life care tools for relevant professionals across the acute hospital setting. Collaborating with colleagues across the ICS in coordinating the delivery of this facilitation & education Research and Audit Ensures clinical effectiveness by critically appraising current research and applying it to practice Develops and participates in appropriate nursing and medical research Regular audit of issues relevant to current service provision, reviewing and amending practice as a result Co-ordinates the development implementation and review of relevant nursing policies procedures and documentation ensuring a consistent evidence based approach to patient care Personal and Professional Responsibilities Maintains a programme of personal and professional development by attending in service training and utilising relevant educational opportunities Functions effectively as a specialist practitioner in line with the recommendations of the Nursing and Midwifery Council and the Department of Health and operates as a member of the Palliative Care Team and wider multidisciplinary teams Uses clinical supervision to maintain and improve practice To be alert to ones own psychological health needs with reference to working in palliative care and the needs of others and to be able to receive and offer support when needed Work with the service manager in developing service policy and delivery participating in projects education and consultation as appropriate Maintain high professional standards of care and professional expertise Using a palliative care database keep accurate records submit statistical returns and compile reports as required Standard Clauses: The purpose of this post should remain constant but the duties and responsibilities may vary over time within the overall role and level of the post The post holder may from time to time be asked to undertake other reasonable duties Any such changes will be made in discussion with the post holder in the light of service needs. Competence The post holder is responsible for limiting their actions to those which they feel competent to undertake If the post holder has any doubts as to their competence during their duties then they should immediately speak to their line manager or supervisor Codes of conduct All employees of the Trust who are required to be registered with a professional body to enable them to practise within their profession are required to comply with their code of conduct and requirements of their professional registration Those staff that are not required to be registered with a professional body are required to comply with the Trusts codes of conduct
