Main Purpose As a Health Care Assistant you will work within a multi-disciplinary team providing clinical care and support to hospice patients and their families To maintain effective communication skills to deliver emotive information clearly, sensitively and unambiguously with staff, patients, families and professionals by face-to-face, telephone and/or electronic means. To work collaboratively as part of the multi-disciplinary team (MDT) team sharing the clinical caseload of patients/carers within the locality/in-patient unit for patients who have palliative care needs to ensure efficient and timely care is delivered. To work as part of the MDT, seeking advice from colleagues and/or clinical leadership team when required, in order to deliver excellent palliative and end of life care. To support and participate in any streamlined locality/IPU processes and services by working to agreed quality improvement standards including clear outcome measures to produce efficient and highly performing clinical services. To support the Hospice in building strong relationships with external partners and stakeholders and promote the good reputation of the Hospice e.g. local trusts, CCG, health and social care partners, businesses and voluntary organisations. To ensure that the service delivers an excellent standard of care to patients and their families. To actively promote patient and family/carer involvement in care. To act under the direction of the Locality Care Team Manager and professional colleagues and in accordance with written guidance and procedures, undertake agreed clinical activities with designated patients. To monitor patients for any changes in symptoms and provide non-pharmacological relief where possible, referring for professional advice when required. To provide personal care, pressure area care, mouth care etc.as per the personal care plan. To be flexible and creative in the care of the patient,and family. Involve and enable the family and friends to continue caring for the patient and support them as appropriate. To provide emotional support and communicate effectively with patients and their family/carers To work unsupervised in the patients home as required.
