We are on an ambitious journey to achieving a truly co productive approach to working with individuals and their carers. We believe that everyone has strengths, and our mission is to support individuals to maintain their independence so that they can live their lives well, we believe this starts at the point of first contact with the department. Our “Strengths-Based Model of Practice” supports a multidisciplinary approach to enabling and supporting people who lives with barriers caused by physical ill health, mental health, frailty, and ageing.
We are looking to appoint a full-time occupational therapist who will be based within the Early Intervention and Prevention Hub, which includes the short-term social work, community support workers and the front door response in Adult Social Care. The post holder will support with:
Triaging/screening of incoming referrals,
Adopt a solution-focused approach to promote independence,
Ensure responses are proportionate, person centred, and aligned to the individual’s strengths and capabilities,
Making effective use of existing personal and community resources.
By using specialist OT skills and knowledge, the post holder will undertake evidence based and outcome focussed assessments to optimise care delivery and ensure the level of commissioned support is proportionate to the individual’s needs. They will take responsibility for own and the Therapy Technician’s caseload and provide support to the wider team to promote and implement the outcome focussed and strengths-based methodology.
The postholder will adhere to the professional and practice standards laid down by the Royal College of Occupational Therapists, by the Health and Care Professions Council and Bridgend County Borough Council.
The ability to greet customers through the medium of Welsh is a requirement for this post.
Protecting children, young people or adults at risk is a core responsibility of all council employees.
An Enhanced with Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.
