We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. About this role…

We are looking for a Lead Worker to join the City Navigators team.



The Lambeth Living Well Network Alliance supports people in Lambeth experiencing mental illness. Thames Reach works in partnership with South London and Maudsley NHS Foundation Trust, Lambeth NHS Clinical Commissioning Group, Lambeth Council and Certitude to deliver mental health services as part of the innovative transformation of mental health services in the borough.

We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.

You will:

.Line manage Lead Workers and co-manage the CLaSS MDT
.Lead on Multi-Disciplinary Meetings across the Trust
.Screen and allocate Adult Social Care referral cases
.Attend professional meetings and ward rounds to facilitate timely discharges
.Lead on Trust delayed discharges and those in Hospital Step Down’s
.Liaise with clinical teams around patient continuity of care in the community

You will have:

.Experience in multitasking and prioritising competing demands
.Experience in working in a fast paced and demanding role
.Proven experience in continual improvement of service delivery, ensuring a high-quality outcome-focussed service which involves all internal and external stakeholders and meets targets in line with key performance indicators.
.Knowledge and experience of working with vulnerable adults with severe and
.An understanding of what constitutes a barrier to discharge from hospital and how to overcome these.
