Key Responsibilities The post holder will work as an autonomous General Practitioner within a supportive primary care team, providing high-quality, patient-centred clinical care. You will independently assess, diagnose, and manage a wide range of health conditions, supported by a multidisciplinary team across the practices and PCN. You will provide expert advice and guidance to patients, carers, and colleagues, promoting evidence-based practice and maintaining clinical excellence at all times. The role also involves contributing to the education and training of medical trainees, students, and other healthcare staff,supporting continuous learning and professional development. As a GMC-registered practitioner, you will be professionally accountable for all aspects of your clinical practice, ensuring patient safety, effective record-keeping, and the highest standards of care. This is an excellent opportunity to develop your skills and experience within an innovative and forward-thinking PCN Main Duties Conduct face-to-face and telephone consultations, home visits, and manage patient queries, reports, and correspondence efficiently. Triage patients presenting to the practices via telephone, online, or in person. Make professional, autonomous clinical decisions for self-referred patients or referrals from colleagues. Assess and manage undifferentiated and undiagnosed patient problems. Screen patients for disease risk factors and early signs of illness. Develop care plans with patients in line with practice protocols, promoting healthy lifestyles and patient education. Manage long-term conditions and patients with complex needs. Maintain clear and contemporaneous consultation notes to agreed standards. Collect data for audit and comply with QOF requirements. Deliver enhanced services as required. Manage and review patient test results. Refer patients to secondary care or other relevant services, including internal PCN team members. Prescribe and issue repeat and acute prescriptions in line with local and national guidelines.
