About the role

Co-ACTION is a five-year NIHR-funded research programme developing and testing a personalised, multi-sectoral support model for people living with multiple long-term conditions (MLTCs).

The programme takes a holistic, bio-psychosocial approach, combining social and self-management support to improve wellbeing and reduce inequalities. The Co-ACTION intervention is being evaluated through a mixed-methods trial across several sites in England.

This exciting post combines intervention delivery with applied health research, offering the opportunity to make a tangible difference in people’s lives while contributing to national research aimed at transforming integrated care.

As an Intervention Facilitator, you will deliver the Co-ACTION intervention with participants in the Plymouth area, providing personalised, flexible support to individuals with multiple long-term conditions and their carers. You will work closely with GP practices, NHS providers, community organisations, and voluntary sector partners to coordinate care and ensure support is truly centred around participants’ needs.

You will be based within a local health or social care organisation and supported by both the local provider and the research team. Alongside direct work with participants, you will contribute to data collection, reflective practice, and the evaluation of the programme, helping to shape the ongoing development of the Co-ACTION model.

You will:

• Deliver the Co-ACTION intervention to participants within a defined local area
• Be key in building trusting and supportive relationships with people living with MLTCs and their carers, co-developing person-centred support plans
• Provide proactive follow-up, anticipatory care planning, and support through key life and care transitions
• Work collaboratively with GP practices, community services, and voluntary sector partners to coordinate holistic care
• Engage with participants in their real-life contexts, considering the social, financial, and community factors that influence wellbeing
• Maintain accurate digital care records and ensure safeguarding and risk management in line with protocols.

What we’re looking for

We are looking for someone who is passionate about improving people’s lives and reducing health inequalities. You will have experience working directly with people in health, social care, or community settings, alongside a strong commitment to person-centred, relationship-based support.

This is an opportunity to be part of a nationally significant programme transforming care for people with multiple long-term conditions. Also to work at the interface of research and practice, gaining skills in both intervention delivery and applied health research. You will receive high-quality supervision, training, and support from experienced researchers and practitioners. Importantly, you will make a meaningful impact on individuals’ lives and contribute to tackling health inequalities in the community.

This is a part-time position working 29.6 hours per week on a fixed-term basis for 22 months due to funding.

Due to the nature of this role, an Enhanced Disclosure and Barring Service Check plus an Adult’s Barred List check will be required.

For this role, you will need to be on site 1 days a week. Although we can accommodate working from home for this role, flexibility to be on-site will be expected due to business needs.
