Summary
Competencies and Training: To be involved in the planning, implementation, delivery and evaluation of clinical and essential training using relevant national and local guidelines and frameworks. Support in delivery of training programmes led by other clinical educators. Implement, participate, and facilitate in the development of Practice Supervision as peer groups for all staff groups across the Community Nursing teams. Facilitate the development of a peer learning groups for all staff groups across theCommunity Nursing teams. Develop opportunities to enhance development and sign off, of clinical practicecompetencies. Facilitate and support in the sign off, of clinical competencies, supporting evidencebased, quality leadership. Support with the delivery and implementation of essential clinical training for thesestaff groups. Provide expert clinical leadership and support to all staff groups across thecommunity Nursing teams. Student Nursing Associates and nurses acting as a role model in provision of cleardirection for nursing and practice development. The role includes a balance of clinical, educational and leadership responsibilitieswhich may vary according to need and to be agreed by the Practice lead for nursingto support service development. Responsibility for financial and/or physical resources Ensure training equipment and consumables are maintained. Monitor stock levels and liaise with training team in regards ordering and restocking Maybe required to instigate ordering of new or updated equipment in line with current guidelines and best practice. Responsible for the safe use of equipment other than equipment which they personally use; And/or responsible for maintaining stock control and/or security of stock. Responsibility for Workforce and Operational Issues: Works within the NMC Code of Practice. Ensure quality service delivery by developing and reviewing of workforce competencies in collaboration with professional training and development, practice lead for nursing and operational mangers. Support on strategies to support Professional training and development and Practice lead in the development of suitably skilled staff by using workforce planning developments to support the development of sill mix competency and development within the staff groups identified. To actively participate in supporting new ways of working and developing staff quality and competence within clinical practice. Develop opportunities to enhance development and sign off, of clinical practice competencies Remain updated and competent and ensure that clinical practice is evidence based. Have an up-to-date personal development plan and professional portfolio Participate in an annual appraisal of their work in line with the Knowledge and Skills Framework (KSF) where the job description will be reviewed, and objectives set. In line with the annual development plan the post holder will be expected to undertake any training or development required to fulfil their role. Responsibility for people who use our services Provide nursing knowledge and experience to any member of the multi-disciplinary team in relation to patient care, ensuring a visible presence is maintained within clinical areas. Set, agree and monitor standards for clinical practice and patient care and ensure teams have the right skills to deliver a high-quality service. Ensure changes in clinical practice are reflected in training and competency assessment, alerting the team manager and practice lead for nursing where there is an unmet need. Work autonomously in the community setting, recognising the risks associated with lone working, e.g., aggressive behaviour, and work proactively to reduce those risks. Work as an autonomous practitioner, managing and prioritising own workload according to local guidelines and identified need. Working in a variety of community settings, exercising a high level of judgement, discretion and decision making in clinical care. To facilitate and support effective clinical assessment and transfer those skills to support quality of care and clinical competence for those identified staff in line with clinical assessments listed below: To Support and perform a comprehensive holistic assessment and physical examination for patients using clinical skills and knowledge to identify problems or symptoms. To be able to determine if symptoms warrant further investigation and/or treatment and initiate this as appropriate. To plan, implement and evaluate on an ongoing basis, care delivery using evidence based best practice. To be able to identify and resolve complex clinical issues. Support staff in providing expert clinical care and health promotion interventions that will help prevent unnecessary hospital admissions and periods of crisis. This will include the ordering and interpreting of diagnostic tests and act on results, referrals to relevant clinicians using clinical guidelines and policies to support clinical decision-making and the prescribing of medicines via independent and supplementary prescribing arrangements. To initiate and support medicines management incidents within our community teams linking with professional lead to identify themes and support with training. To support and facilitate effective Work with the multi-disciplinary team across health and social care to co-ordinate the development, implementation and evaluation of training. Responsibility for implementation of policy and/or service developments To be involved in the planning, implementation, delivery and evaluation of clinical and essential training using relevant national and local guidelines and frameworks. Responsible for the day-to-day support for ongoing development of quality and evidence based clinical practice development across the Community Nursing Service. Ensure changes in clinical practice are reflected in training and competency assessment, alerting the team manager and Practice lead for nursing where there is an unmet need. Ensure staff are competent to complete delegated duties. Develop and facilitate Peer support groups for the identified staff. Lead on the development and implementation of practice supervision for the identified staff group. Support the Practice lead for nursing and Team managers to plan and develop future trends in workforce development and develop workforce plans. Provide support for all staff groups across the community Nursing teams to allow them to mentor and support within clinical practice. Responsible for the ensuring clinical support and training provided within the role is in line with the implementation of relevant policies and guidance. Support competency development for all grades of nursing and support staff in line with national standards/care pathways. Participate in regular supervision, clinical and line management, engage in reflective practice and be committed to continued development. Identify own learning needs and seek opportunities to develop knowledge and expertise. Maintain an individual professional portfolio as direct evidence of clinical practice at an advanced level ensuring that this portfolio meets revalidation criteria and keep abreast of current developments within field of practice. Constantly seek to extend the boundaries of practice within the speciality, stressing the nursing contribution and identifying areas where clinical practice will lead to improvements in service users care. COMMUNICATIONS AND RELATIONSHIPS The post holder will be required to have excellent, effective, communication skills to support staff development and enhance quality care. Be able to use a range of teaching styles to meet the needs of learners with consideration to neuro diversity and reasonable adjustment to ensure equity and accessibility for all. Maintain and foster good relationships with professionals and non-professional colleagues concerned with the provision or development of healthcare services. The post holder will be required to use a computer, being responsible for timely recording patient activities for IT data collection, dealing with e-mail queries, action development plans. Lead and participate in team activities so as to develop and consolidate a cohesive and supportive team ensuring openness within the team. You will be able to communicate complex, sensitive or contentious information. This may be because agreement or cooperation is required or because there are barriers to understanding. The Post Holder to be supplied with a mobile phone and for work purposes and be expected to abide by the organisation policy. Develop and maintain effective and appropriate communication networks with other members of the Primary Health Care Team and other agencies. Reporting and recording of all incidents and near misses as per policy. Have good communication skills to facilitate effective communication with staff, patients, carers including sensitive and accurate information. The post holder will require need to actively participate in effective information sharing of development areas within clinical competencies undertaken by staff
