Fixed-Term for 18 months due to funding.
We are seeking a dedicated and compassionate Housing Solutions Lead to join our team at the Changing Futures Hub, supporting individuals facing rough sleeping and complex life challenges.
This pivotal role involves assessing the housing and wider support needs of individuals experiencing homelessness, ensuring all interventions are compliant with the Homelessness Reduction Act and delivered in a trauma-informed, person-centred manner.
Working in close partnership with the Rough Sleepers Outreach Team, you will conduct outreach visits to individuals sleeping rough, completing comprehensive assessments that consider not only housing needs but also the multiple and often unmet needs related to mental health, substance misuse, physical health, offending history, and social exclusion.
We are looking for someone with:

Excellent interpersonal skills, empathy, and the ability to build trust with individuals who may be disengaged from services.
Proven experience in the homelessness sector, with a commitment to trauma-informed and person-centred practice.
Sound knowledge of housing legislation, pathways, and support services.
