SCOPE AND RANGE Manage a defined group of patients under the indirect supervision of a Registered Nurse or registered practitioner, using evidence based, patient centred principles to undertake a range of delegated tasks independently. Works within the limits of own competence and Nursing Associate scope of practice. Is responsible and refers issues beyond scope of practice to the relevant registered staff members. Identify personal development needs that ensure consolidation of knowledge, skills, and competency to undertake the care and treatment of responsibilities assigned. To delegate work to support worker staff as required. To supervise and line manage band 2/3 staff and undertake the ongoing supervision of the routine work of others. Take responsibility for the training of others and may be required to deliver training. To demonstrate an understanding and application of professional, local, and national guidelines and legislation relating to the Nursing Associate role and health and social care provision. Responsibility for People Management Demonstrate those inter-personal skills that promote clarity, compassion, empathy, respect and trust. Contribute to team success and challenge others constructively.. Act in ways which support equality and value diversity. Demonstrate own duties to new or less experienced staff. Contribute towards developing a culture of learning and towards developing high quality learning environments. Develop skills in relation to coaching-teaching individuals-carers-other staff. Acts as an excellent role model by upholding and implementing good practice in the workplace. To be actively involved in team effectiveness by attending staff meetings and contributing as a team player to team development. Support and supervise junior members of the team as required - to participate in providing instruction, learning opportunities and assessment of Bands 2/3 staff, students and other trainees as required. Support and supervise pre-registration nursing students as per the NMC guidance for supervision and assessment and other learners within the team. To recognise and understand different roles and responsibilities of individuals working in Livewell Southwest. To delegate work to others as required. Take responsibility for the training of others and may be required to deliver training. Responsibility for financial and-or physical resources To use and maintain resources efficiently and effectively and to provide evidence of these efforts. Exercise personal duty of care in the safe use and storage of equipment. Be environmentally aware and prudent in use of resources and energy. Safely carry out moving and handling of individuals and equipment. Use electronic resources to input, save and retrieve information making use of eRecord systems. Responsibility for administration Prioritises workload effectively, this could include scheduling your workday appropriately or managing your own diary of appointments and visits ,as well as mileage if applicable, This requires a high level of time management and organisation skills. Ensure clear, concise, accurate and legible documentation and all communication is maintained in relation to care delivered adhering to local and national guidance. To undertake any non-clinical - clerical duties as necessary. This may include maintaining stock levels, ordering equipment, and filing, dealing with telephone calls, and cleaning. To participate in audit as part of service review and improvement processes. To comply with legislation, policies, procedures, and other quality approaches relevant to the work being undertaken. To participate in the planning and implementation of operational policies, projects, and service development within the team. Follow all information governance guidance and policies. Maintain confidentiality as outlined within data protection policies. Report any accidents or incidents as per organisational policy Responsibility for people who use our services To give compassionate, holistic evidence-based practice to patients in the in line with national and organisational approved policies / procedures and individual care plans. Develop, implement, and evaluate individual care plans under the appropriate delegation of a registered professional. Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment. To record and report regularly on patient progress and treatment to the MultiDisciplinary Team and handovers. To demonstrate a working knowledge and understanding of conditions relevant to the clinical area. Observes any changes in the patients condition, manages these changes in line with scope and competence, reporting findings to the most appropriate clinician in a timely manner. In the event of a change in patient status, can safely decide whether to proceed with treatment, including medicines administration and-or seeks support from a registered nurse to decide. Undertakes delegated clinical skills within their role having once undertaken relevant training, deemed competent and adhere to standard operating procedures. Safely uses a range of non-invasive and invasive interventions whether therapeutic, technological, or pharmacological, within the defined scope of the role. Implements the goal planning process and outcome measures used in the clinical area to review progress of the patient. Administers medication within scope of practice, competency and as delegated by the registered nurse. Support patients to improve and maintain their mental, physical, behavioural health and wellbeing. Active involvement in the prevention of and protection against disease and ill health Recognise and report any situations, behaviours or errors that could result in poor care outcomes. Report to appropriate registered care professional information received from the individuals, carers and members of the team. Ensure all patient related information is treated sensitively and always adhere to the principles of confidentiality. Prioritise the day-to-day management of own caseload/ workload delegating activities to junior staff as appropriate. Engage in public health, community development, and in the reduction of health inequalities and increased self-management. To act as the patient's advocate and, by providing information and support, facilitate the patient's own choices in conjunction with carers and other agencies where relevant. Respect patient privacy, confidentiality, dignity, wishes, and beliefs always minimising any necessary discomfort. Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other registered care professionals as appropriate. Recognise issues relating to safeguarding vulnerable children and adults and report any problems or concerns to the appropriate registered care professional. Assist in the assessment of and contribute to the management of risk across several areas within the care environment. Develop a working knowledge of other providers resources and referral systems to ensure individuals needs are met, within scope of practice. Demonstrate good understanding of the Mental Capacity Act - Deprivation of Liberties and apply principles to everyday practice seeking advice / guidance from the Registered Nurse as required. Document all care given in accordance with Livewell Southwest policy. Contribute to the implementation and monitoring of clinical standards and outcomes. Report concerns regarding health and safety issues within the working environment to the appropriate staff. Demonstrate a high standard of care within a Quality and Safety framework. Responsibility for implementation of policy and/or service developments To be responsible for collecting and reporting statistical data in line with LSW policies and procedures. To contribute to the planning, evaluation and audit of practice, clinical pathways and protocols within the clinical setting. To participate in research and clinical audit as appropriate keeping up to date with changing practices. Participate in audits and surveys relating to practice or patient satisfaction as required. To understand how to apply changes in legislation, policies and procedures. Adhere to legislation, policies, procedures, and guidelines both local and national. To be aware of the Health and Safety aspects of your work and abide by policies including the prompt recording and documentation of incidents and accidents. Share ideas with colleagues to improve care. Contribute to the improvement of service by reflecting on own practice and supporting that of others. To be accountable for own clinical practice. Advocate the role of the Nursing Associate Carry out responsibilities and act professionally in line with The NMC Code (2018) Take responsibility to ensure NMC revalidation and registration requirements are met as per NMC guidelines, seeking support if required to complete.
