Main Duties and ResponsibilitiesClinical Practice Provide expert clinical assessment, diagnosis, treatment, and management for patients presenting with undifferentiated or complex needs. Work autonomously within professional boundaries to deliver safe, evidence-based care in primary, urgent, and community care settings. Initiate and interpret investigations, prescribe medications (if qualified), and make independent clinical decisions as appropriate. Support patients to understand and manage their conditions, promoting health education, self-care, and lifestyle modification. Recognise and manage acute and chronic conditions, escalating appropriately when required. Respond effectively to emergency situations, providing immediate and safe clinical interventions. Maintain accurate, contemporaneous patient records in accordance with professional standards and information governance policies. Service Delivery Work flexibly across BCA services, including Streaming, Enhanced Access, and future projects. Contribute to maintaining and improving the quality and efficiency of clinical services. Deliver care aligned with NICE, local, and national guidelines. Support service evaluations, audits, and data collection to monitor outcomes and improve patient care. Participate in implementing new care pathways and innovative service models. Provide clinical leadership and mentoring within the team as required. Leadership and Teamwork Act as a professional role model, promoting excellence in advanced clinical practice. Work collaboratively with multidisciplinary teams to ensure integrated, coordinated care delivery. Support the induction, training, and supervision of new or junior staff members. Contribute to a positive team culture that values learning, reflection, and professional development. Engage in local meetings and contribute to the continuous improvement of BCA services. Risk Management and Governance Comply with BCA and NHS policies for clinical governance, risk management, and safeguarding. Maintain up-to-date mandatory training and professional revalidation. Identify and manage clinical risks, escalating concerns as appropriate. Participate in audits, significant event analyses, and clinical incident reviews. Promote infection prevention and control and ensure adherence to health and safety standards. Communication Communicate sensitively and effectively with patients, families, and carers, ensuring understanding and consent. Deliver difficult or sensitive information in a professional and compassionate manner. Maintain clear, respectful communication with colleagues, demonstrating teamwork and collaboration. Use clinical systems (e.g., SystmOne, Cerner) effectively for record-keeping and communication. Ensure information sharing complies with confidentiality and data protection requirements. Learning and Development Take responsibility for ongoing professional development and revalidation. Participate in BCA clinical supervision, appraisals, and protected learning activities. Support and mentor students or other healthcare professionals as appropriate. Engage in reflective practice and shared learning across services.
