Are you passionate about supporting Care Leavers to thrive as they transition to independent living? Do you have experience working with young people with complex needs? If so, we’d love to hear from you!
We are looking for 4 part time Family Practitioners (Emotional Wellbeing) to join our dedicated Leaving Care Teams in Staffordshire. This role focuses on providing targeted emotional wellbeing support to care leavers aged 18+, helping them build resilience, regulate emotions, and feel empowered.
This role may involve occasional evening work and could involve travel to any district in Staffordshire, also sometimes could involve travel out of county. We do offer a flexible working policy.

Main ResponsibilitiesAs an Emotional Wellbeing Practitioner, your main responsibilities will include:
- Work alongside young people's allocated PA's to support young people to meet the aims of their Pathway Plans
- Work closely with care leavers, building trusting relationships and offering one-to-one support as well as some group work when needed
- Complete emotional wellbeing assessments
- Contribute to multi-agency support plans
The Emotional Wellbeing Practitioner is an exciting new role that we are pleased to be able to offer to support our Care Leavers. As this is a new role, there will certainly be scope for this role to evolve and change over time, as the service demands and we will encourage you to contribute to the growth and development of this role, in co-ordination with our Leaving Care Senior Practitioners.
The Ideal CandidateIf you’ve worked with young people facing mental health challenges, trauma, or emotional distress, your experience is highly valued. You’ll need to be compassionate, resilient, and skilled in building rapport with vulnerable individuals. Creativity is key in this role, and we encourage innovative approaches such as arts-based, digital, and experiential techniques to engage young people meaningfully. Your support will help them navigate life’s challenges, develop emotional regulation skills, and strengthen their sense of identity.
This is a flexible role that may involve occasional evening work and travel across a double district or out-of-area. You’ll be part of a supportive team committed to making a real difference in the lives of care-experienced young adults.
