Birmingham Community Healthcare NHS Foundation Trust (BCHC) is seeking an enthusiastic and motivated Admissions and Discharge Support Worker to join our specialist stroke and neuro rehabilitation ward at Moseley Hall Hospital.

This is an exciting opportunity to be part of a multidisciplinary team providing high-quality, patient-centred care for individuals recovering from stroke and neurological conditions.

Please note:This postdoes not meet the eligibility requirements for sponsorshipunder the UK Skilled Worker visa route. Applicants will need to demonstrate their right to work in the UK.

As an Admissions and Discharge Support Worker, you will play a vital role in ensuring smooth and timely patient journeys into and out of our service. Working closely with nursing, therapy, medical, and social care colleagues, you will:

Support the coordination of patient admissions, ensuring accurate documentation and effective communication with patients, families, and referrers.

Contribute to discharge planning from the point of admission, helping to identify and address potential barriers early.

Liaise with internal teams, external agencies, and community services to support safe and efficient transfers of care.

Provide patients and families with information and reassurance during the admission and discharge process.

Maintain accurate records, trackers, and discharge documentation to ensure compliance with Trust standards.

Assist the ward team in monitoring and escalating delays, supporting the Trust’s commitment to timely flow and excellent patient experience.

About You

We are looking for someone who is:
-Organised, proactive, and able to work in a fast-paced environment.
-A confident communicator with strong interpersonal skills.
-Able to build positive relationships with patients, families, and professionals across health and social care.
-Experienced in an administrative, support, or patient flow role (NHS experience is desirable but not essential).
-Passionate about improving patient experience and outcomes in rehabilitation care.
