The overall job purpose of the Dementia and delirium Nurse Specialist istoact as an expert resource for ward teams / departments in order to support them in the delivery of effective care for patients with dementia and delirium. They will be responsible for proactively reviewing patients admitted with confusion either from dementia or delirium, to assess the cause and advise on management for clinical teams As a leader they will be responsible for ensuring a good understanding of dementia and delirium throughout the organisation, to ensure patients and carers receive a high standard of care. The Dementia and delirium nurse specialist will review the pathways relating to dementia and delirium to determine the key stages where s/he will have interventions to influence patient care. This includes contributing to the implementation and monitoring of assessment tools to ensure optimum safety and care in relation to dementia practice. This will require the person to adopt a highly visible and accessible approach for patients, their relatives/carers and staff. Leading by example s/he will empower staff in their personal and professional development to undertake a greater range of clinical skills to modernise and improve patient care, including uptake of training relating to dementia practice. The Dementia anddeliriumnurse specialist will review the nature of assessment and care planning which takes place across the organisation in relation to dementia and delirium. With particular attention on the stages when the patient may be transferred between services. The Dementia anddeliriumnurse specialist will be a source of knowledge and expertise, providing support to staff in their day to day management of patients who present with dementia. Excellent communication skills are essential to this role. Support the delivery of quality key performance indicators and evidence the impact relating todelirium and ordementia care, including the experience for carers. The post holder will be expected to play a proactive role in quality and service improvement and working closely with the Clinical matron and multi-disciplinary team will contribute to the monitoring and auditing of clinical standards of care within the defined specialty.
