Summary
Deliver high-quality, autonomous care to patients within the Virtual Ward, using advanced clinical assessment and decision-making skills. Monitor patients remotely using a variety of digital platforms; therefore,strong IT skills and digital literacyare essential. Identify and manage patients suitable for step-up or step-down care between acute, urgent, and community settings, including in-reach to Accident & Emergency and acute hospital wards. Promote public health and wellbeing through theMaking Every Contact Countapproach, supporting individuals, carers, and families to make positive changes to their physical and mental health. Support and mentor junior staff, contributing to their development and the overall capability of the team. Developing and delivering teaching sessions to junior members of the team. Actively participate in service improvement initiatives, research, and audit activities to enhance clinical practice and patient outcomes. Accountabilities: Ensuring safe and effective clinical practicethrough evidence-based care and adherence to professional standards. Enhancing the patient experienceby delivering compassionate, responsive, and personalised care. Contributing effectively to the organisations objectives, including integrated care delivery and admission avoidance. Efficient and effective use of resources, ensuring value for money and sustainability. Ability to request and interpret clinical investigations (i.e, bloods, ECG, etc). Flexibility in working across the Virtual Ward, Urgent Community Response, and Accident & Emergency Department, supporting seamless transitions of care under the front-door model.
