Crynodeb
You will follow treatment plans for patients with acute exacerbations of chronic diseases, long term conditions and support the treatment of patients to prevent them being admitted to hospital. You should have excellent clinical, written, and verbal communication skills, be able to work independently and manage your own workload. For purposes of the role, you must hold a full UK driving licence, have good IT skills and have excellent communication skills and be able to meet the travel demands required.
