DUTIES AND RESPONSIBILITIES CLINICAL 1. To maintain regular and close communication with physiotherapists and team leader. 2. To work alongside the MDT to deliver programmes of care and therapy for treatment, as per plan of care determined by physiotherapists. 3. To develop a rapport with patients and encourage interest and participation in their treatment programme. 4. To support and undertake the assessment and management of non-complex patients rehabilitation needs under the direct/indirect supervision of the physiotherapist. This will include initial data collection, assessment of mobility/basic function, implementation of identified treatment plans and information, monitoring/analysis of patients progress, feedback discussion with seniors, provision of written reports and completion of appropriate MDT referrals. 5. To support and assist the physiotherapists within the medical and surgical teams in the treatment of complex patients. 6. To assist with implementing exercise programs and progressing/maintaining physical ability to optimise mobility and independence for surgical, medical and long stay inpatients including those undergoing stem-cell transplants. 7. To undertake stair assessments with ambulatory patients prior to discharge to ensure safety and report any concerns to seniors 8. To identify and supply appropriate walking aids to patients 9. To identify appropriate manual handling aids for the safe moving and handling of patients under direct/indirect supervision of senior physiotherapist. 10. To provide appropriate post-op exercise and lymphoedema advice/information to surgical patients following surgery. 11. To measure and refer out to patients own local wheelchair services for the provision of appropriate wheelchairs and pressure cushions. 12. To attend relevant ward MDT meetings, Board rounds and handover to identify appropriate new referrals and provide feedback of follow-up patient progress for the physiotherapy and occupational therapy team 13. To update relevant electronic waiting lists and daily worklists to assist with the organization organisation and prioritization prioritisation of the team workload. 14. To participate in audit and research projects within their own area of responsibility. 15. The post holder will recognize recognise precautionary signs/symptoms and contraindications and will seek the support of an appropriate qualified member of the team when the need arises. 16. To participate in a rotation scheme with the band 4 between surgery, and medical oncology wards, which may include haematology oncology and teenage and young adult wards as required. PROFESSIONAL 1.Record and monitor relevant patient data. 2.Maintain excellent communication with patients, relatives and carers on their treatment plan and progress. 3.Demonstrate a variety of communication skills for patients and relatives who have limited or restricted communication. 4.Demonstrate excellent customer care skills as a role model to others. 5.Provide effective verbal or written feedback to therapists regarding patient care. 6.Liaison with outside agencies and wider members of the multidisciplinary team 7.Participate and maintain Continual Professional Development 8.Maintain a personal development plan using a personal portfolio to ensure clinical practice is based on best practice and up to date. 9.Participate in appraisal with senior staff on an annual basis. 10.Keep up to date with all policies, procedures and protocols related to the Rehabilitation Unit and relevant clinical practice. 11.Attend staff meetings and In-service training, as required. 12.Complete mandatory training and timely updates as required. 13.Identify risk issues that impact on the patients rehabilitative needs. 14.Demonstrate high standards of clinical practice at all times. 15.Demonstrate effective team working inclusive of all relevant professionals. 16.Be accountable for own practice. 17.Keep up to date with current practice pertinent to the NHS and professions. ORGANISATIONAL 1.Record, report and where required take responsibility for a variety of systems required for delivering the service e.g., equipment, supplies, and environmental estates issues 2.Be responsible for relaying confidential information as directed by qualified therapists. 3.Use clinical databases e.g., Careflow, CWP/electronic patient records, order systems, etc. 4.Participate in departmental projects as directed by senior staff. 5.Take responsibility for the maintenance of up-to-date community referral information kept within the department. 6.To undertake regular stock-check and ordering of all mobility aids, splints and braces (including spinal) to maintain a regular stock within the rehabilitation department. 7.To order individual collars/braces/splints at the request of the physiotherapist. 8.To maintain the Rehabilitation/gym environment in an organized and tidy manner. 9.To ensure the continued cleanliness of all equipment within the Rehabilitation environment alongside the rehabilitation team. 10.To keep the information folders up to date and well stocked.
