Crynodeb
The post holder`s key role is to support adults with intellectual disabilities to access mainstream health services. The post is within Newark & Sherwood locality within Nottinghamshire. The post holder will participate in the delivery of enhanced training to primary care colleagues around the annual health check and reasonable adjustments for people with intellectual disabilities. Ensure those aged 14+ who meet the eligibility are included on the annual health check register.
